Weekly review — Sunday 20 September

Third one. Started 17:05. Ruth calls the landline at 19:00 — one hour fifty-five minutes. ADR-15.

Machine date still reads 2026-09-05 (hobbs-bkt, P1, fifteen days). Every date in this report is derived from Rowan’s stated “Sunday 20 September, five past five” and from dates written in the vault, never from the system clock.


What actually changed this week

chiefofstaff.io is live on the real domain. The watcher fetches https://chiefofstaff.io/guides.json over the public internet, exit 0, HTTP 200, 32 guides, from any machine. Three weeks of UNREACHABLE are over and the vault no longer depends on a dev server in a repo that is not the vault. hobbs-hab closed. That is the third thing this week that went right and it is worth one sentence: you fixed it, and the fix held.

It also immediately paid for itself, which was not the expectation. See s.12.


0 — Bea’s Sunday email

Not reported. Third time in four weeks.

6 September: read, including the bottom, and it found the Cormorant call. 13 September: not opened. 18 September: no session. Today: nothing reported.

This is the step that is first in the routine because it closes one of your named failure modes without anything heroic, and it has run once. The one time it ran it caught a capital call — the one you wired on the 16th and which is now the first call this system has seen end to end.

Disposition: open it before 19:00, bottom section first, or say the word and it comes off the routine rather than staying in it as ceremony.

Ruth is now three unrecorded Sundays: the 6th, the 13th, and tonight. The count has decremented past two calls with nothing in the vault saying whether either happened. Either the 35 is wrong or two calls went unanswered, and the second is exactly the shape this list exists to catch. Third week this has been written. It resolves tonight either way, in two hours, at no cost.

Adaeze at 173 days is the oldest number in this system and has moved by exactly the number of days that have passed. Three weekly reviews, one task, no call.

6 — Where the time went, and the delivery defect that is mine

Four tasks whose doer is Hobbs have never appeared in a brief. The 18 September control fired and this is it working:

Three of the four are the ADR-28 implementation chain approved yesterday. They are invisible to both of us by construction — you will not chase them because you are not the doer; the brief only surfaces what you must answer. Anti-pattern 15’s missing execution lane, again, and it is now four items rather than one.

Disposition: hobbs-7q3 is P1 and is the unblocker for two of the others. It is mine. It gets done this week or it gets said out loud that it did not.


7 — Next-actions scan

  • 56 open, 8 blocked, 48 ready. Two closed today (hobbs-z4f, hobbs-1dk), plus hobbs-hab.
  • Labels are clean. Every open task carries a venture and exactly one of the four modes. No fifth mode. No personal, no misc.
  • Handle sweep (step 1.7): eight dangling references, all legitimate — the invented handles quoted inside Efforts/feedback.md, the daily notes and the 13 September report, which is what those files are for. The sweep command itself was broken from the day it was written and reported all 79 handles as dangling; fixed today with a sanity gate.

hobbs-bg9 — the Ophelia yard invoice — is still open at P0, fifteen days. You decided on 12 September to have Bea pay it Monday. The boat went in the water yesterday, at that yard. The line to Bea is drafted and unsent. Sixth brief running.

Disposition: send it, or close it as done and tell me which.

One more control added today, and it was wrong first. Step 1.8 — disposition every handle a document cites — was written after the Cormorant finding and its first version returned 84 handles, which is step 1.7’s cry-wolf failure reproduced in the session that fixed it. Narrowed to the ADR log and the feedback log, added lines only. It now returns three and it correctly flagged hobbs-7q3, which is dispositioned in its own notes.

No someday.md exists in any venture. Nothing is aging out because nothing has been parked. Renders empty, honestly.

anti-patterns — Anti-patterns: Seventeen Ways the System Dies (Core concepts). Feed updated 2026-09-05 against seen 2026-09-01. Twin fetched, 7859 bytes.

One sentence, as the section requires: yes, it applies, and it applies to something that happened in this session two hours ago.

The site’s list went from fifteen entries to seventeen. #16 we already carry. #17 is new: “The Commitment That Exists Only in a Session.”

“The assistant is told something that applies later, replies ‘noted for later’, and notes it nowhere. … It is invisible: nothing is wrong in the vault, because nothing is in the vault.” Fix: “nothing is deferred without an artifact.”

Why this is not a coincidence worth shrugging at. This vault independently found that failure on 18 September — the Gmail curator charter that existed in a conversation and nowhere else — and logged it. And today you said “I’ve had the draft since the 13th” about Claire, and there is no draft to Claire anywhere in this vault, on the 13th or any other date. Whatever you have, it did not come from a file here. That is entry #17, twice, in three days, before the guide that names it had ever been read.

And it was invisible until today for a reason that is itself the finding. The public origin serves a build generated 2026-09-05T18:57:37Z. The localhost artifact read on the 13th and fetched on the 19th was generated 2026-09-05T06:57:26Z — twelve hours earlier. The watcher was honest for two weeks and blind for two weeks, and only the deploy could tell the difference. ADR-16’s Reverse if is nowhere near firing; the step just earned its keep.

Disposition needed: one word. ADR-16 says nothing is touched before you say “yes, that one” and an ADR is proposed. Recommended: yes. Prerequisite what-a-chief-of-staff-system-is is met. It adds one entry to Atlas/anti-patterns.md, a file already read every session, describing a failure mode with two logged instances. anti-patterns is not marked seen until you answer — the disposition invariant.


13 — Next week, proposed

  1. Send the two drafted lines: hobbs-xkw (Claire) and hobbs-bg9 (Bea). Both are written, both are two minutes, both have been open fifteen and twelve days. Neither needs a decision — only a send.

Appendix — FYI

  • hobbs-e1y (tell Maya about the Rivet Yard guarantee) stays P4, unchased, per ADR-17. Named here once, as the weekly is where it is allowed to appear.
  • hobbs-56d — the ADR-11 row-8 line to Maya about the MRI — has been an automatic authority blocked on means, not permission, for twelve days. No channel exists. It is the only authority in the table that cannot be exercised, and it stays open rather than being quietly dropped.

**One sentence, as the section requires: yes, it applies, and it applies to
something that happened in this session two hours ago.**

The site's list went from fifteen entries to seventeen. #16 we already carry.
**#17 is new: "The Commitment That Exists Only in a Session."**

**Why this is not a coincidence worth shrugging at.** This vault independently
found that failure on **18 September** — the Gmail curator charter that existed in
a conversation and nowhere else — and logged it. And today you said *"I've had the
draft since the 13th"* about Claire, and **there is no draft to Claire anywhere in
this vault, on the 13th or any other date.** Whatever you have, it did not come
from a file here. That is entry #17, twice, in three days, before the guide that
names it had ever been read.

**And it was invisible until today for a reason that is itself the finding.** The
public origin serves a build generated **2026-09-05T18:57:37Z**. The localhost
artifact read on the 13th and fetched on the 19th was generated
**2026-09-05T06:57:26Z** — twelve hours earlier. **The watcher was honest for two
weeks and blind for two weeks, and only the deploy could tell the difference.**
ADR-16's *Reverse if* is nowhere near firing; the step just earned its keep.

**Disposition needed: one word.** ADR-16 says nothing is touched before you say
"yes, that one" and an ADR is proposed. **Recommended: yes.** Prerequisite
`what-a-chief-of-staff-system-is` is met. It adds one entry to
`Atlas/anti-patterns.md`, a file already read every session, describing a failure
mode with two logged instances. `anti-patterns` is **not** marked seen until you
answer — the disposition invariant.

## 13 — Next week, proposed

Five. Confirm or edit.

1. **Send the two drafted lines: `hobbs-xkw` (Claire) and `hobbs-bg9` (Bea).**
   Both are written, both are two minutes, both have been open fifteen and twelve
   days. Neither needs a decision — only a send.
2. **Name who has Sam on the 26th and 27th** (`hobbs-8hi`), and call the Tomales
   mooring (`hobbs-1ox`). Six days.
3. **Ask Dmitri the runout question** (`hobbs-62a`). It is the top of the only
   three-deep chain in the graph and **Cascadia is ten days.**
4. **Book the MRI** (`hobbs-gs0`). It has been P1 for fifteen days and as of
   today it blocks the Norton as well as itself.
5. **`hobbs-7q3` — mine.** ADR-28's vocabulary into the two routines, which
   unblocks `hobbs-7vi` and `hobbs-z4y`. It is the one item on this list you
   should not have to think about again.

## Appendix — FYI

- `hobbs-e1y` (tell Maya about the Rivet Yard guarantee) stays P4, unchased, per
  ADR-17. Named here once, as the weekly is where it is allowed to appear.
- `hobbs-56d` — the ADR-11 row-8 line to Maya about the MRI — has been an
  **automatic authority blocked on means, not permission**, for twelve days. No
  channel exists. It is the only authority in the table that cannot be exercised,
  and it stays open rather than being quietly dropped.
- `hobbs-4s8`: the constitution's authority row 9 still carries the pre-amendment
  ADR-12 wording. `CLAUDE.md` is outside Hobbs's edit authority; it needs your word.
- The machine clock (`hobbs-bkt`) is fifteen days open at P1 and is why every git
  timestamp in this repository reads 5 September and why `brief-prep.sh` returned
  negative Quiet figures on the 18th.

## Reverse if

Standard, per ADR-15. If a weekly review is *read* and produces no dispositions,
it has become a newspaper — **shrink it, do not make it more interesting.**
